To save time, you may complete these forms before coming to the Parking Services office.
- Parking Application: Application covering the following categories: employee, retired employee, faculty, student (on- & off-campus), carpool, non-affiliate, non-employee/non-student, and clean air vehicle.
- Appeals Form: Used to appeal a parking citation.
- Reappeals Form: Used to reappeal a parking citation.
- Payroll Deduction Form: Pre-Tax Payroll Deduction form used by University employees to deduct parking fees directly from their payroll.
- Employee Fee Chart: Chart showing salary-based permit rates for employees.
- Special Request Form: Used to request parking in a lot which is unavailable for sale.
- Department Recharge Form: Form to bill departments for purchases made at Parking Services (Departmental use only).
- Special Needs Brochure: The University of Massachusetts Amherst provides handicapped - accessible parking in all its lots in compliance with American with Disabilities Act (ADA) regulations.
